BRIEF HISTORY OF MEDICAL CENTER

 

The Federal Polytechnic Offa Medical Centre started operation in September, 1992 at the mini-campus of the Polytechnic with 8 (Eight) members of staff. The Polytechnic moved to its permanent site 2012 and since then the center has been providing medical services in the two campuses i.e mini and main campus.

 

The Medical centeris aNHIS (National Health Insurance Scheme) accredited center which provide services for 24 (Twenty-four) hours all through the week to all the members of the polytechnic community and runs three shifts daily, namely: morning, afternoon and night shifts.

UNITS IN THE MEDICAL CENTER

 

There are several units in the medical center and they work hand in hand for the smooth running of the center. They include:

  1. Health Information Unit
  2. Nursing Unit
  3. Laboratory Unit
  4. PharmacyUnit
  5. General Outpatient and Inpatient Unit

 

  1. HEALTH INFORMATION UNIT

This unit analysis and protect digital and traditional medical information vital to patient care. The unit plays information system, develop health policy and identify current information needs in the Polytechnic. This unit also provides documentation of health care servicesto a patient with the directive of a physician, Nurse or other care giver. These records included documentation of medical observations, opinions of professional as well as the treatment and diagnosis documentation.The health information department is dedicated to maintaining patient medical records and keeping health information confidential according to medical rules and regulations.

 

CHARACTERISTICS OF HEALTH INFORMATION UNIT

  1. All new students, ND1/HND1 part-time and full-time, IJMB and Degree student must register at them medical record office to acquire clinic cards.
  2. All new students should come along with one passport and acceptance receipt for registration and all students should attach their passport photographs to the clinic card and come with an extra passport when visiting for the first time.
  3. All students; old or new intakes; must come along with their medical records card when coming for treatment at the school clinic.
  4. All students must come to the record office before seeing the doctor.

ACTIVITIES OF HEALTH INFORMATION UNIT

The regular activities of the health information unit include:

  1. Filing System
  2. Creating folder for all patients
  3. Retrieval and Storage of files
  4. Data collection and Daily record keeping
  5. Documentation and presentation

 

HEALTH INFORMATION UNIT PATIEN’TS CATGORIES

In the Health Information Unit students are categorized mainly into two, these are

  1. Patients diagnosed and treated for malaria
  2. Patients treated for other clinical conditions such as STIs (Sexually Transmitted Infections), UTIs (Urinary Tract Infections), Peptic Ulcer Diseases, Road Transport injuries, HIV, AIDS, Sickle Cell Anemia etc.

 

  1. NURSING UNIT

This unit is a special unit typically in a hospital settings dedicated to patients with specific needs. Nurses in this department are trained to care for specific situations or types of patient. It also focuses on the care of individuals and families so they may attain, maintain or recover optimal health and quality of life.

There are 4 (Four) sub-units in Nursing Unit, they are: General Nursing Practice, Family Planning, Immunization and Health Education

 

GENERAL NURSING PRACTICE

In the Medical Center, the patient vitals (Blood pressure, height, temperature, pulse, weight, respiratory rate) are taken by nurses before the doctor’s consultation. Following the doctor’s consultation, the patient presents him / herself to the nurses for parenteral drug administration, wound dressing and admission if necessary.

FAMILY PLANNING

The family planning services are the ability of individual and couples to anticipate and attain their desire numbers of children and also the spacing and timing of their births.

The following family planning methods are available in the Medical Center. They are:

  1. Male and female condoms
  2. Implants
  3. Injectable
  4. Combined oral contraceptives

 

IMMUNIZATION

This unit is a key component of primary health care and an indispensable human right. It is a global health and development success in saving millions of lives every year. A vaccine reduces risk of getting a disease by working with the body’s natural defenses to build protection.

The following vaccines are available to administer to staff and students at the Medical Center. They are:

  1. Tetanus toxoid
  2. Meningitis vaccine
  3. Hepatitis A, B & C vaccines
  4. Yellow fever vaccine
  5. Albedazole tablet
  6. Mectizan tablet
  7. AstrazenecaCovid-19 vaccine

The Polytechnic Management in collaboration with the Kwara State Ministry of Health made available the AstrazenecaCovid-19 vaccines to the members of the Polytechnic community. 117 (One hundred seventeen) individuals were immunized on the 23rd of March, 2021 and the second dose will be administered on 21st of July, 2021.

HEALTH EDUCATION

Nurses in the Medical Center plays a huge role in illness prevention and health promotion in the Polytechnic, they also have a key role in providing information in the form of health taking. Nurses in the Polytechnic are easily accessible for counseling and health promotion.

 

  1. LABORATORY UNIT

This unit in Medical Center is where tests are carried out on clinical specimens to obtain information about the health of a patient to aid in diagnosis, treatment and prevention of diseases.

On resumption, fresh students (N.D, H.N.D, IJMB and Degree) are expected to register at the laboratory unit. Before full laboratory process commences, the students are counseled to avoid rowdiness in the school clinic. In both campuses, Mondays to Thursdays is for tests while Fridays is for collection of results and medical certificate.

The following tests are conducted on each student:

  1. Blood group and Genotype
  2. Packed Cell Volume (PCV)
  3. Mantoux screening for tuberculosis.

At least, one hundred samples are taken from students daily.

 

  1. PHARMACY UNIT

The pharmacy unit is a unit in the medical centre which deals solely with drugs and clinical consumables. Pharmacy is the next point of call after consultation. The unit has qualified pharmacists and they stock good quality NAFDAC approved drugs directly purchased from pharmaceutical companies.

The pharmacy mainly dispenses the available drugs to patients who come to the unit with their formal prescription from the doctor.

GENERAL ACTIVITIES OF THE PHARMACY UNIT

On the first working day of each month of the year, the stock of drugs and consumables were taken and recorded in the improvised bin card. The pharmacy unit attends to patients with prescriptions written for them. Patients that are attended to include the pupils of the primary and secondary schools of the polytechnic, the IJMB students, NYSC members posted to the polytechnic, students and staff.

The Pharmacists’ Council of Nigeria (PCN) also visited the unit on inspections in 2014 and 2018 and the report of the visitation was forwarded to the management.

  1. GENERAL OUT-PATIENT AND IN-PATIENT UNIT

The Medical officers (Doctors) are in-charge of this department. The department renders primary medical care to the members of the Polytechnic community. They also play a prominent role in providing medical care for all and all patients are treated to the highest standard of quality medical care. The medical officers open daily, Monday to Sunday from 8am to 4pm and provide 24 hours emergency services.

 

ACTIVITIES OF THE CONSULTING ROOM

The activities of the consulting room are:

  1. Questioning of the patient about his / her medical and health problem.
  2. Study the past medical record and drug history of the patient.
  3. And thorough physical examination is carried out on the patient.

 

TERTIARY INSTITUTIONS SOCIAL HEALTH INSURANCE PROGRAMME (TISHIP)

The Tertiary Institutions Social Health Insurance Programme is a social security system whereby the health care of students in tertiary institutionsis paid for from funds pooled through the contributions of students. It is a programme committed to ensuring access to qualitative healthcare servicefor students of tertiary institutions thereby promoting the health of students with a view to creating conducive learning environment.

This important programme commenced in the Federal Polytechnic, Offa in 2016 and being monitored by the TISHIP Management committee.

 

TISHIP MANAGEMENT COMMITTEE

TheTISHIP Management Committee commenced in the Federal Polytechnic, Offa in 2016 and being reporting periodically to the Institution authority. Thecommittee is headed by the Medical Director andrepresentatives from the Health Maintenance Organization (HMO), Student's body, Student's Affairs,Bursar, Legal Department and Secretary.

 

ROLES OF TISHIP MANAGEMENT COMMITTEE

The roles of TISHIP Management committee include:

  1. Oversee the implementation of TISHIP in the institution.
  2. Ensure that the HMOs meet their obligations to students.
  3. Act as a key stakeholder in quality assurance and monitoring.
  4. Liaise with the student population to ensure that their needs arebeing met.
  5. Provide regular feedback to the management of the tertiary institution.
  6. Keep records of the activities of the scheme.
